Answer: If you have provided liquidity into a Polygon Pool about a year ago, you should be able to see and manage your share in the pool. To do this, you need to go to the Pool Withdraw page on Curve's interface (either the old or new one). You should see a red button labelled "Migrate". Click on this button, and you should be able to see and manage your share in the pool. You can also view your liquidity on Zerion, so this should help you to confirm that your liquidity is still there. Once you have clicked on the Migrate button, you should be able to see and manage your share in the pool.
